View, download, or send your health summary.
Generate Your Health Summary
Click Documents in the header.
Select the Health Summary tab.
Enter a from and to date to choose the date range for your health summary.
Click Save Document.
View and Download
To view, click View Health Summary.
The top portion of the health summary will include your demographics, your provider's information, and their practice location details. The bottom portion of the summary includes your detailed medical record, separated into sections. Click the X in the upper right to close the health summary view.
Click Download Health Summary.
Copy the password displayed on the page. Your health summary will be downloaded to your device as a secure .zip file that requires this password. Health summary electronic files have 256-bit encryption. You will need to use an unzip tool that can support this type of encryption, or you may receive an error when you enter the password for the file. Click here to download a tool that supports this encryption.
Send by Email
Click Email Health Summary.
You are prompted to enter the recipient's email, add a subject for the email and a brief message. When done, click Send.
