Log in to the patient portal and check the notifications in the Action Required section. Here you will be notified if you have consent forms that need to be signed. Click the link for You have consent form(s) to sign to jump directly to the consent forms section. — Log in to the patient portal and check the notifications in the **Action Required** section. View Consent Forms

Click Documents in the header to see consent forms without using the quick link. Next, click the Consent Forms tab at the top menu. — Click **Documents** in the header to see consent forms without using the quick link. Next, click the **Consent Forms** tab at the top menu.

Sign Form

Click Sign to the right of the form that you want to complete. — Click **Sign** to the right of the form that you want to complete.

Check the Box to confirm that you agree to sign electronically, then enter your name in the box below. Click Sign to complete the consent form. Your provider will automatically be notified that you have completed this form, and it will be sent to them electronically. — Check the **Box** to confirm that you agree to sign electronically, then enter your name in the box below.   Consent forms are set up by your provider's office. If you have questions about a particular question or intake form, contact your provider directly.

View Completed Forms

Completed consent forms will show without a Sign button.

Click the PDF icon to view or download your completed consent form.
